REPORT DUE BY APRIL 15, 2025
There are a few ways to submit the Grant Completion Report:
Option 1: To submit online through Kaleidoscope, login here. We will email you the direct link as well.
Option 2: The following information can be emailed to info@lauzier.org or mailed to the Lauzier Office PO Box.
- NAME OF ORGANIZATION
- CONTACT PERSON
- NAME OF PROGRAM/PROJECT
- Provide a summary of the work funded by the Lauzier Grant. Explain any variances which may have occurred from the original grant proposal.
- Provide a financial report on the expenditure of funds. Any unspent funds should be explained. Also, please explain if the Lauzier Grant helped your organization attract additional financial support if applicable.
- Explain the degree to which organizational objectives have been realized. Include factors which contributed to, or impeded, your success.
- Discuss the impact of the program/project on your organization and/or the community it serves.
- If applicable, explain any projected future plans.
- Feel free to include additional comments, photos or documentation as appropriate.
Please contact our office if your organization needs an extension of the grant term or if any funds remain unspent. The Foundation can provide authorization for an alternate use of remaining funds if necessary. It is our preference that grant funds be spent for the benefit of the organization rather than returned.
